Requirements for 3-6 business days processing and for Next day service processing
To use our service you MUST fill out the ONLINE Department of State passport application below, for Step 1 THEN click the Create button at the bottom and THEN print the form.
If you do not use the form below your passport application will be delayed.
Step 1. DSP-11 passport application (must do Online) Note: Please make sure to mark US Passport Book and not US Passport Card when filling out the form.
Step 2. DSP-64 gray lost/stolen form (must do Online)
Step 3. Lost/Stolen Statement(must print out)
Step 4. Release letter (must print out)
Step 5. Print our Passport order form (must print out) and send a copy to us inside your Federal Express Priority Overnight, also in the Federal Express should be a Check for $109.00 for our service fee plus return delivery fees.(Note: If you pre-paid for your passport when you did your birth certificate thru us then disregard our fees) If you need a same day service please call our office.***
Step 6. Original Certified Birth Certificate, or naturalization certificate, or the most recent passport.
Step 7. Two Passport Photographs taken by a passport photographer.
Step 8. Valid Picture Driver's License issued more than 6 months ago. If your Driver's License has been issued or renewed in the last 6 months, you will need to provide at least 3 personal documents.
Step 9. 1 check made out to Department of State for $170.00 for adults and $140.00 for children and another check for $25.00 for the post office/county clerks fee.***
Step 10. To have your passport expedited then you will also need proof of departure, which will be ONE of the following:
a) copy of your airline tickets or
b) travel itinerary or
c) company letter of expedite (sample)
In order for us to process your lost or stolen passport, you must first get all the requirenments listed above then go to a local acceptance location ( Every person must appear in person as of 6-17-04 ) which will either be a special post office or county clerk's office. You will need to show the items listed above, which will then be put into a government sealed envelope. Make sure you get the envelope back from the clerk, and do not open it.
Before you go to your local acceptance location (Either a special post office or county clerk's office) please call us at 713-974-1645 or 888-736-2692 so we can go over your information with you.We will make sure everything is in order. After you appear at your local acceptance agent you will then Federal Express your sealed envelope, order form and payment to us at:

Inside the FedEx should be:
1. Sealed Envelope
2. Our Order form filled out completely (step 3 above)
3. Payment to US Birth Certificate unless you pre-paid*
4.******Please write the applicant's full name, date of birth and phone number on the sealed envelope********
