Consular Report of Birth (FS-240):
On November 1, 1990 the Department of State ceased issuing multiple copies of the Consular Report of Birth (FS-240). As of that date, a new format for the FS-240 went into effect. All previously issued FS-240 are acceptable proof of U.S. citizenship (Public Law 97-241-Aug. 24,1982)
To obtain a replacement for a lost or mutilated document, please submit a written request** including a notarized affidavit concerning the loss of the FS-240 and both order forms completely filled out with a $160 fee****, payable to U.S. Birth Certificate.Net, to the address below. (No personal checks, only money orders or credit cards accepted) The affidavit ** must contain the name, date and place of birth of the subject, a statement regarding the whereabouts of the original FS-240, and be signed by the subject, parent or legal representative.
