Requirenments for 3-6 business days processing
To use our service you must fill out the Online Department of State passport application below, for Step 1 ,then click the Create button at the bottom and ,then print the form.
If you do not use the form below your passport application will be delayed.
In order for us to process your passport, we must receive the following information from you:
Step 1. DS-5504 name change form(must do online form)
Step 2. Please fill out the order form (must print out) and print a copy to send to us inside your Federal Express Priority Overnight etter, also in the Federal Express should be a Check for $184.00 or $244.00 (Next day) for our service fee plus return delivery fees (see order form) (must print out). If you need a next day service please call our office as we will need to fax a different release letter to you.
Step 3. Authorization letter (must print out)
Step 4. Your passport, which must still be valid.
Step 5. Proof of your name change, such as: (originals only)
a) marriage license or
b) divorce decree or
c) court order
Step 6. Government fee - $60.00 (included in totals above)
Step 7. To have your passport expedited , then you will also need proof of departure, which will be ONE of the following:
a) copy of your airline tickets or
b) travel itinerary or
c) company letter of expedite (sample)
